Method of Preparation:

  I have used the series specific author package to prepare my manuscript. *Strongly recommended* Author packages: http://www.ams.org/authors/procpackages.html

OR

I have not used the author package to prepare my manuscript, but I have verified that the type block measures 5 x 8.4 inches (12.7 x 21.4 centimeters or 30 x 50.5 picas).   NOTE - Please include running heads when measuring text.  Do not include page numbers at the bottom of a page in the measurement. NOTE that the first page of the text including the title should measure only 6.5 inches (16.5 centimeters or 39 picas) in length, measured from the top of the title.

General Formatting:

I confirm that the type size of the text is 10pt (default setting in the author package).

I confirm that all magnification commands (AMS-TeX) or \documentclass size options (LaTeX) and (if I am using the series specific author package) all changes to page dimensions have been removed.

I confirm that the 2010 Mathematics Subject Classification numbers (both primary and secondary) are included as an unmarked footnote on the first page. NOTE - See http://www.ams.org/msc for classifications.

I confirm that the academic (or other) affiliation of author(s) is included at the end of the paper following the bibliography or reference section.

Grant Information:

I confirm that grants or contracts supporting the research are cited in an unmarked footnote on the first page.

OR

No support was received.

Permission:

I confirm that written permission has been granted and received for ANY reprinted material including text, figures or photographs appearing in the paper; a copy of any permission letters are enclosed.  I understand that permission must be free of any restrictions which may limit the AMS’s ability to produce, publish, promote, and distribute the volume in which my paper will appear.  I understand that any restrictions may result in my paper being omitted from the volume.

Final Submission (requires two steps):

The AMS requires both electronic and hard copy submission of all accepted manuscripts as specified below.

I confirm that I have thoroughly proofread my paper prior to submission to the AMS.  I understand that the AMS will not proofread or provide proof copy prior to publication.

Step 1 — Electronic submission:

I confirm that the .tex, .dvi, and .eps (if applicable) files of the FINAL version (as accepted by the editor) have been submitted using ONE of these methods:

via the AMS website (http://www.ams.org/submit-book-journal) *Preferred method*.  Note - you will receive a confirmation email message when the files have been successfully uploaded.  

OR

by FTP to the AMS web site; if this method is used, you must send an email message about the submission  (including information about the volume and, if appropriate, its editor) to pub-submit@ams.org in order for your paper to be processed. THIS IS REQUIRED.

OR

are on the enclosed CD

Step 2 — Hard copy submission:

Hard copy printed from the FINAL .dvi file (as accepted by the editor) is enclosed.  This is required for all papers.

A cover letter indicating the book series and volume editor, providing phone, fax, and email for the corresponding author, is enclosed.

The cover letter, hard copy, and completed author checklist should be mailed to:
